dc.description.abstract | Hospital-acquired infections caused by adaptive resistance mechanisms and enhanced virulence in pathogens, often within the challenging context of biofilms, have led to the development of alternative treatments. Bacteriophages, viruses that infect bacteria, have emerged as a potential alternative or complement to antibiotics, especially in complex cases involving multi-species biofilms. This thesis examined the combined effect of phages and antibiotics, investigating how it contributes to biofilm eradication while mitigating antibiotic resistance. It is crucial to note that phage-antibiotic combinations can sometimes hinder treatment, emphasizing the importance of understanding optimal timing, dosage, and combinations for maximizing therapeutic potential. Phage therapy shows promise due to its bacteriaspecific targeting and ability to penetrate biofilm layers. However, understanding how biofilm formation conditions influences the treatment out-come is very important for an effective standardization of in vitro phage/biofilm interaction studies. The initial study was focused on the effect of hydrodynamic conditions of biofilm formation on phage killing activity. Results revealed variations in structure of P. aeruginosa biofilm formed under dynamic and static conditions, and a better phage performance against biofilms formed in dynamic conditions. These results demonstrate the need to develop and adopt standard conditions to assay phage/biofilm interactions. Furthermore, the study evaluated the antibiofilm ability of a newly isolated P. aeruginosa phage (EPA1) and seven antibiotics against mono and dual-species biofilms. Simultaneous phage-antibiotic application significantly enhanced biofilm reduction, compared to individual treatments, while sequential application (first phage, then antibiotic) led to impressive biofilm eradication. Antibiotics concentration and the timing were pivotal factors influencing the effectiveness of combined treatment. To validate these findings, further experiments were conducted using a threedimensional lung epithelial model and an artificial wound model. Sequential phages and gentamicin treatment eradicated P. aeruginosa biofilms in the lung model. Similarly, phage EPA1 and SAFA with GEN in sequential and multiple dose administrations effectively reduced biofilms of both P. aeruginosa and S. aureus. Simultaneous multiple dose administration showed the most promising results against the dualspecies biofilms. In conclusion, phage and antibiotic combination holds promise in controlling infectious biofilms. However, a careful consideration of optimal application strategies, including timing, dosage, and order of administration, is necessary to maximize the efficacy. Future research efforts should focus on elucidating these complex interactions to develop optimized therapeutic strategies against biofilmassociated infections, ultimately improving patient outcomes. | por |
dc.description.abstract | A elevada prevalência de infeções hospitalares causadas bactérias patogénicas com elevadas resistências aos antibióticos e virulência associada, designadamente a capacidade de formação de biofilmes têm levado à procura crescente de mecanismos de controlo alternativos. Os (bacterió)fagos, vírus que infectam bactérias, surgiram como uma alternativa ou complemento aos antibióticos, especialmente no caso do tratamento de infeções crónicas associadas a biofilmes. O trabalho desenvolvido no âmbito desta Tese, avaliou o efeito combinado de fagos e antibióticos no controlo de biofilmes formados por populações mistas de Pseudomonas aeruginosa e Staphylococcus aureus. É sabido que a combinação fago/antibiótico pode, em determinados casos, dificultar o tratamento, enfatizando a importância de compreender o momento ideal, a dosagem e as melhores combinações para maximizar o potencial terapêutico. Por outro lado, torna-se também relevante compreender os mecanismos de interação fago-biofilme sob diversas condições de formação de biofilme. O trabalho inicial incidiu sobre o estudo da influência das condições hidrodinâmicas na capacidade que os fagos apresentam no controlo de biofilmes de P. aeruginosa. Os resultados revelaram variações na estrutura do biofilme formado em condições dinâmicas e estáticas, tendo o fago uma capacidade anti-biofilme superior nos biofilmes formados em condições dinâmicas. No âmbito do trabalho desenvolvido foi caracterizado um fago (EPA1) e sete antibióticos no controlo de biofilmes. A aplicação simultânea de fagos e antibióticos aumentou significativamente a redução do biofilme, comparando com a ação de cada um destes agentes de forma isolada. No entanto, quando esta aplicação foi sequencial, verificou-se a erradicação do biofilme. A concentração dos antibióticos e o intervalo de aplicação revelaram-se fatores cruciais que influenciam a eficácia do tratamento combinado. Para validar os resultados obtidos em modelos in vitro, foram feitos ensaios num modelo tridimensional de epitélio pulmonar e num modelo de ferida artificial. A aplicação sequencial de fagos e gentamicina erradicou biofilmes de P. aeruginosa no modelo pulmonar. Da mesma forma, o fago EPA1 e o SAFA com gentamicina em administrações sequenciais e de múltiplas doses reduziram significativamente ambos os biofilmes de P. aeruginosa e S. aureus. A administração simultânea de fagos e antibióticos em múltiplas doses revelou ser a mais promissora. Em conclusão, a ação combinada de fagos e antibióticos tem elevado potencial no controlo de biofilmes mistos. No entanto, é necessário considerar cuidadosamente as estratégias de aplicação, incluindo tempos de aplicação, dosagem e ordem de administração, para maximizar a eficácia. Esforços futuros de pesquisa nesta temática, devem concentrar-se em elucidar essas interações complexas para desenvolver estratégias terapêuticas otimizadas contra infecções associadas a biofilmes. | por |
