Resumo(s):	This document presents the model of the questionnaire (in English) applied in the context of the Landscaping Open Government Data Use doctoral thesis. This instrument was developed to collect data about the use of open government data by the public sector. It was applied to two population groups: the Brazilian states and federal district public administrations and the public agents in public institutions in Brazil.
