Resumo(s): | Atualmente, um dos propósitos da Optometria é a avaliação do estado refrativo do
olho humano e em termos clínicos essa prática é revista através do exame subjetivo.
Contudo, um dos atuais objetivos para a comunidade científica tem sido a criação de uma
ferramenta que seja capaz de estimar a refração de um sujeito, quando executada pelo
mesmo de forma autónoma.
A partir de equipamentos de medida da refração objetiva, bem como, dos
conhecimentos das aberrações do olho humano, que permitam estimar a métrica de
medida do estado refrativo surgiu este estudo para se avaliar a viabilidade do EyeQue
PVT™, um instrumento baseado na obtenção da refração ocular através de um dispositivo
móvel.
Os principais objetivos deste trabalho foram avaliar a repetibilidade do EyeQue
PVT™ comparando os resultados do mesmo com os do autorrefratómetro de campo
aberto e de um aberrómetro. Baseado no valor das aberrações do olho humano foram
estimadas as métricas de medida, em função do diâmetro pupilar, que mais se assemelham
à técnica recorrida para o estudo.
Foram avaliados 60 olhos de 30 sujeitos com idades compreendidas entre 18 e 24
anos (20,47±1,74 anos) e um valor médio da esfera registado com o autorrefratómetro de
campo aberto de -0,50±1,43D. Registaram-se os valores de refração objetiva através de
cinco medidas com o autorrefratómetro (Grand Seiko WAM5500), com recurso ao
EyeQue PVT™ e à sua aplicação móvel instalada num telemóvel Apple iPhone 8 Plus
com um sistema operativo iOS 12.1, obteve-se a refração de cada paciente em ambos os
olhos. Através de medidas registadas previamente com o aberrómetro estimou-se a
métrica de medida do estado refrativo que mais se assemelha ao equipamento em estudo.
Em termos clínicos a diferença do valor da esfera encontrada com a aplicação
móvel em estudo mostra um valor ligeiramente mais miópico em comparação com o
equipamento de refração objetiva comercial utilizado, em média de -0,20D, sendo que as
diferenças não são significativas, mesmo com outras métricas em estudo. Contudo, estes
valores revelam uma grande dispersão, sendo que o intervalo de confiança de 95% varia
entre 1,84D e -2,16D. A repetibilidade de medidas em sujeitos míopes apresenta
diferenças clinicamente significativas e em casos de miopias altas registam-se valores da
esfera com desvios hipermetrópicos. Em suma, concluímos que o dispositivo móvel EyeQue™ apresenta uma baixa
precisão na medição do erro refrativo, embora tenha uma boa exatidão. Nowadays, one of the purposes of optometry is the evaluation of the refractive state of the human eye and in clinical terms this practice is reviewed by subjective exam. However, one of the current objectives for the scientific community linked to the area will be the creation of a tool that is able to estimate the refraction of a subject, when executed by the same one in an autonomous manner. Therefore, the challenge will be to replicate, objectively, the subjective criterion of the individual. Using measurement equipment of the objective refraction as well as of knowledge of the aberrations of the human eye that allow to estimate the metric of measure of the refraction this study appeared to evaluate the viability of EyeQue PVT ™, an instrument that arose as response to the challenge described. The main objectives of this study are to evaluate the viability of the EyeQue PVT ™ by evaluating the results of the same with those of an open field autorefractometer and through the knowledge of the aberrations estimate the measurement metric, depending on the pupil diameter, which most resembles the technique used during the study. The measures were perfomed in 60 eyes of 30 subjects with ages ranged between 18 and 24 (20,47±1,74) years with a mean value of the sphere registered with the open field autorrefarctometer of -0,50±1,43D. Objective refractive values were recorded using five measures with the Grand Seiko WAM 5500, using the mobile application of the EyeQue™ installed on an Aplle iPhone 8 Plus mobile phone with Ios 12.1 operating system was obtained the refraction of each patient in both eyes. Through previous measurements registed with the aberrometer was estimated the refractive metric that most likeky resembles the equipment under study. In clinical terms, the difference in the value of the sphere found with the mobile application under study shows a slightly myopic value in comparison with the commercial objective refraction equipment used, on average of -0.20D, and these differences are not significant, even with other metrics under study. However, these values reveal a great dispersion, with the 95% confidence interval varying between 1.84D and -2.16D. The repeatability of measurements in myopic subjects presents clinically significant differences and in cases of high myopia values of the sphere with hypermetropic deviations were recorded. Therefore, we conclude that the EyeQue ™ mobile device has low accuracy in refractive error measurement. |
