Resumo(s): | A Perturbação Depressiva Major (PDM) é considerada a principal causa de disfuncionalidade a nível global. Atualmente, as terapêuticas mais comuns para a PDM são a Psicoterapia, a Psicofarmacologia, ou a combinação de ambas. No entanto, uma grande porção de pacientes não beneficia das opções terapêuticas disponíveis. A Estimulação Transcraniana por Corrente Contínua (ETCC) é uma técnica neuromodulatória não-invasiva que permite a estimulação ou inibição da atividade neuronal espontânea, com eficácia no tratamento da PDM quando aplicada de forma isolada. Para estudar a eficácia do Tratamento Combinado (TC) de ETCC com uma Intervenção Psicológica (IP) foi realizada uma Revisão Sistemática. Apenas 2 artigos sugeriram maior eficácia do TC. Estes partilhavam de algumas comunalidades: a montagem usada, esponjas com área de superfície superior, e maior eficácia do TC foi apenas visível nos follow-up. Foi realizado um estudo piloto para testar a eficácia do TC de ETCC com Psicoterapia Cognitivo-Comportamental (PCC). 2 participantes com PDM foram incluídos no estudo. A intervenção teve uma duração de 6 semanas em que foram aplicadas 18 sessões de ETCC e 12 de PCC. Foram reportadas melhorias clinicamente significativas na sintomatologia depressiva, sintomatologia ansiogénica e na satisfação com a vida. Estas melhorias foram mantidas nas avaliações follow-up. Major Depressive Disorder (MDD) is the leading cause of disability worldwide. The most common therapeutic options are Psychotherapy, Psychopharmacology, or a combination of both. However, only a small number of MDD patients find these treatments effective in reducing depressive symptoms. Transcranial Direct Current Stimulation (tDCS) is a neuromodulatory technique that stimulates or inhibits spontaneous neural activity, effective in the treatment of MDD when applied in an isolated form. We conducted a Systematic Review in order to study the efficacy of the Combined Treatment (CT) of tDCS with a Psychological Intervention (PI). Only 2 studies found CT to be superior to other forms of treatment. Both shared some commonalities: electrode placement, sponges with a larger surface area, and greater efficacy of the CT was only visible on the follow-up. Next, a pilot study was conducted to test the efficacy of CT of tDCS with Cognitive-Behavioral Psychotherapy (CBT). 2 participants with PDM were included in the study. The intervention phase had a duration of 6 weeks, in which were applied 18 tDCS sessions and 12 CBT sessions. Both patients reported clinically significant changes in depressive symptomatology, anxiety and satisfaction with life. These improvements were maintained in the follow-up evaluations. |
