dc.description.abstract | O atual contexto tem colocado o Serviço Nacional de Saúde sob forte pressão. Por um lado, existe a necessidade cada vez maior de prestação de cuidados, incluindo cuidados em Serviços de Urgência e, por outro lado, a necessidade de controlar a despesa em saúde. Assim, a necessidade de tornar o sistema progressivamente mais eficiente e eficaz é premente. Os Serviços de Urgência são unidades altamente especializadas, onde se prestam cuidados de saúde urgentes e emergentes, ou seja, a pessoas em risco de vida ou perda de função. Estas unidades apresentam-se como fundamentais na prestação de cuidados no contexto do Serviço Nacional de Saúde, tentando colmatar algumas insuficiências existentes. Assim, torna-se pertinente identificar e descrever os indicadores que devem fazer parte de um processo de Benchmarking visando a gestão da qualidade, a promoção da melhoria do desempenho clínico dos cuidados de saúde prestados no Serviço de Urgência Por conseguinte, para obtermos a legitimação dos indicadores optámos por recorrer à metodologia de Delphi. Assim, submetemos à apreciação de um grupo de 15 peritos em gestão de Serviços de Urgência uma lista de indicadores identificados na literatura internacional e nacional, no sentido de obter a legitimação ou não destes. Esta metodologia levou a um consenso acerca da legitimação de uma lista de 54 indicadores, nas dimensões: segurança dos doentes (9), satisfação (1), indicadores de produção (10), eficiência (2), intervalos de tempo (15), indicadores relativos a serviços dos quais o Serviço de Urgência pode depender (4) e indicadores relativos às vias verde obrigatórias (13). O consenso foi também atingido no que respeita a considerar como não legitimados 9 indicadores nas dimensões: segurança dos doentes (2), indicadores de produção (1), intervalos de tempo (4), indicadores relativos a serviços dos quais o Serviço de Urgência pode depender (1) e indicadores relativos às vias verde obrigatórias (2). Assim, o nosso estudo chegou a uma lista consensual de indicadores de desempenho dos Serviços de Urgência que poderão fazer parte de um processo de Benchmarking. dc.description.abstract | The current situation has led to a heavy pressure on the National Health Service. On one hand there is a growing need for care, including care in Emergency Department and, on the other hand, the need to control the health expediture. Thus, the need to make it a progressively more efficient and effective system is urgent. The Emergency Departments are highly specialized units, where it’s provided emergency health care, which means health care to people with life-threatening situations or loss of function. These units provide critical care in the context of the National Health Service, sometimes, trying to overcome some shortcomings in the system. Thus, it is pertinent to identify and describe the indicators that should be part of a Benchmarking process, aiming quality management and promoting an improvement of the clinical performance of health care in the Emergency Department. Therefore, we used the Delphi method to obtain indicators validation. So we submitted a list of indicators, identified in the national and international literature, to examination, by a group of 15 experts in Emergency Departments management, to obtain validation or not. This approach led to a consensus on a list of 54 indicators, within the following dimensions: safety of the patient (9), satisfaction (1), output indicators (10), efficiency (2) of time slots (15), indicators for services that the Emergency Department can depend on (4) and indicators relating to required fast pathways (13). Consensus was also reached in which regard to the non-validation of 9 indicators in the dimensions: patient safety (2), production indicators (1), time intervals (4), indicators of the services that the ER can depend on (1) and indicators relating to required fast pathways (2). Hence, our study reached a consensual list of Key performance indicators for the Emergency Department that can be used in a Benchmarking process. In this context, Benchmarking can be a powerful ally in improving the quality and efficiency of the emergency room in Portugal. | por |
