Resumo(s): | Este trabalho tem como objetivo verificar se a iluminação ambiente, presente no consultório, no
momento da medição da pressão intraocular (PIO) com tonómetro de sopro influência o valor
obtido.
Para a realização deste objetivo foi testada uma amostra de 80 olhos, subdivididos em dois
grupos: não-glaucomatosos (70%) e glaucomatosos (30%). Foram realizadas medidas da PIO
com tonómetro de sopro (Reichert AT555) em 3 condições de iluminação diferentes (fotópica,
mesópica e escotópica), e em seguida com tonómetro de Goldmann na condição escotópica. Foi
também registado, no final, a espessura corneal central (ECC).
Na variação da iluminação no consultório, verificou-se a existência de diferenças estatisticamente
significativas no diâmetro pupilar em ambos os grupos, não-glaucomatosos (p<0,001, Kruskal
Wallis Test) e glaucomatosos (p=0,003, Kruskal Wallis Test). No entanto, a variação do diâmetro
pupilar não influência o valor da PIO obtido com tonómetro de sopro nas 3 condições de
iluminação em qualquer grupo não-glaucomatosos (p=0,796, Kruskal Wallis Test) e
glaucomatosos (p=0,953; ANOVA). Não foram encontradas diferenças estatisticamente
significativas entre o valor de PIO obtido com tonómetro de Goldmann e o valor obtido com o
tonómetro de sopro nas 3 condições de iluminação para o grupo não-glaucomatosos (p=0,053,
Wilcoxon Signed Ranks Test; p=0,408, Wilcoxon Signed Ranks Test e p=0,057, Wilcoxon Signed
Ranks Test) e para o grupo glaucomatosos em condições de iluminação fotópicas (p=0,054,
Paired Samples Test). Encontraram-se diferenças estatisticamente significativas para os valores
da PIO obtidos em condições de iluminação mesópicas (p=0,011, Paired Samples Test) e
escotópicas (p=0,46, Paired Samples Test), e também ao comparar a medida da PIO com
ambos os tonómetros em relação à ECC, para córneas com menor espessura no grupo nãoglaucomatosos
em condições de iluminação fotópica (p=0,038, Paired Samples Test) e
escotópica (p=0,028, Paired Samples Test), e para córneas com maior espessura em ambos os
grupos nas 3 condições de iluminação (p<0,050).
Conclui-se que a variação do diâmetro pupilar, obtida com variação da condição de iluminação
(fotópica, mesópica e escotópica), não influência o valor da PIO obtido com o tonómetro de
sopro. Não foram encontradas diferenças estatisticamente significativas no grupo nãoglaucomatosos
entre os dois métodos de medida com qualquer tipo de iluminação testada
(diff<-0,66±2,09mmHg, p>0.053, Wilcoxon Signed Ranks Test ). No entanto, o valor de PIO
obtido com tonómetro de sopro em condições de iluminação fotopicas aparenta ser o que mais
se relaciona com o valor obtido com tonómetro de Goldmann no grupo glaucomatosos
(-1,15±2,77, p=0,054, Paired Samples Test). The aim of this study is to determine if the ambient lighting present at the time of measurement of intraocular pressure (IOP) with air-puff tonometer influence the value obtained. For its realization counted on a sample of 80 eyes, they were divided into two groups nonglaucomatous (70%) and glaucomatous (30%). IOP were measured with air-puff tonometer (Reichert AT555) in three different lighting conditions (photopic, mesopic and scotopic) and after that was taken with Goldman tonometer in scotopic condition. At the end was registered the central corneal thickness (CCT). With the illumination changes, it was found statistically significant differences in the pupil diameter in both groups, non-glaucomatous (p <0.001, Kruskal Wallis test) and glaucomatous (p= 0.003, Kruskal Wallis test). However, the variation of corneal diameter doesn’t influence the PIO value obtained with the air-puff tonometer in the three lighting conditions in nonglaucomatous (p=0,796, Kruskal Wallis Test) and glaucomatous (P=0,953, ANOVA). No statistically significant differences were found between the amount of IOP obtained with Goldman tonometer and the value obtained with the air-puff tonometer in the three lighting conditions for the non-glaucomatous group (p=0,053, Wilcoxon Signed Ranks Test; p=0,408, Wilcoxon Signed Ranks Test and p=0,057, Wilcoxon Signed Ranks Test) and for the glaucomatous group in photopic light conditions (p=0,054, Paired Samples Test). Were found significant differences for mesopic light condition (p=0,011, Paired Samples Test), scotopic light conditions (p=0,046, Paired Samples Test), when comparing the measure IOP with both tonometers in relation to the ECC for corneal thickness in the non-glaucomatous group photopic light conditions (p=0,038, Paired Samples Test) and scotopic (p=0,028, Paired Samples Test), and in thick corneas both groups in the three illumination conditions (p < 0,050). It is concluded that the change in pupil diameter, obtained with varying lighting conditions (photopic, mesopic and scotopic), do not influence the value of IOP obtained with the air-puff tonometer. There wasn’t found statistically significant differences between both methods of measurement with any tested lighting conditions in non-glaucomatous group (diff<-0,66±2,09mmHg, p>0,053, Wilcoxon Signed Ranks Test). However, the IOP value obtained with air-puff tonometer in photopic lighting conditions appear to be most closely related to the value obtained from Goldman tonometer in the glaucomatous group (-1,15 ± 2,77, p=0,054, Paired Samples Test). |
