Resumo(s): | As perturbações do espectro do autismo (PEA) são caracterizadas por uma tríade
de dificuldades ao nível da interação, comunicação e interesses reduzidos mas nos
últimos anos, a investigação também tem dado particular relevância às alterações do
Processamento Sensorial nestas crianças, referindo que entre 42% a 96% das crianças
com PEA apresentam este tipo de disfunção. É neste contexto que se insere a presente
investigação onde se pretende construir um instrumento que avalie a tradicional tríade
que caracteriza as crianças com PEA (Interação, Comunicação e Comportamento e
Interesses), enriquecendo essa avaliação com a inclusão de um novo domínio: o
Processamento Sensorial. Dada a exiguidade de instrumentos de avaliação validados
para esta população, o objetivo geral da tese centra-se na construção, adaptação e
validação de um instrumento de avaliação e intervenção para crianças com PEA na
faixa etária 3-6 anos.
Apresentam-se os procedimentos e os resultados das sucessivas fases de
construção do instrumento, os estudos iniciais junto de pais, profissionais e especialistas
no sentido da compreensão dos itens e da sua relevância na avaliação das diversas
dimensões da escala. Depois desta fase, apresenta-se o estudo piloto realizado com onze
famílias para identificação dos níveis de acordo verificados na classificação dos
diversos itens (indicadores) nas várias dimensões por parte de profissionais com
experiência profissional de vários anos junto de crianças com PEA e descrevem-se os
resultados obtidos numa análise de cariz quantitativa dos itens, resultante da aplicação
de uma primeira versão da escala. Procurando identificar o maior número possível de
crianças com PEA a nível nacional, estende-se o pedido de colaboração às diversas
instituições distribuídas pelas cinco zonas geográficas do país (Norte, Centro, Lisboa e
Vale do Tejo, Alentejo e Algarve). Obteve-se uma amostra nacional de 260 crianças
com PEA; 260 escalas preenchidas pelos profissionais que acompanham estas crianças e
136 instrumentos preenchidos simultaneamente pelas famílias.
Em termos de conclusões, a análise efetuada aos quatro fatores ou dimensões em
análise, mostrou que alguns itens se encontravam distribuídos com saturações aproximadas em dois fatores em simultâneo (por norma nos fatores I e II) surgindo a
―fusão‖ entre a ―Interação Social‖ e a ―Comunicação verbal e não verbal‖, inicialmente
consideradas dois domínios independentes nesta investigação e que se apresenta como
um resultado interessante à luz da investigação atual nas PEA. Este resultado permitiu
anexar itens pertencentes aos dois domínios iniciais e propôs uma ―nova‖ dimensão que
designamos de ―Comunicação Social‖, tal como preconizado nos novos critérios de
diagnóstico constantes do DSM 5.
Ainda relativamente à construção da escala, os resultados reforçam a relevância
e existência autónoma do domínio Processamento Sensorial, incluído na escala, face aos
domínios tradicionais (interação social, comunicação, comportamento e interesses
reduzidos).
A análise da concordância entre o perfil de avaliação da criança com PEA na
faixa etária 3-6 anos avaliado pelos pais e pelos profissionais nas três dimensões, revela
que existem divergências nas avaliações feitas à criança por parte dos pais e dos
profissionais sendo as melhores pontuações atribuídas pelas famílias, sobretudo, no
domínio da ―Comunicação Social‖.
Destaca-se ainda a influencia das variáveis nível socioeconómico e habilitações
académicas dos pais na definição do perfil de avaliação da criança com PEA, na faixa
etária 3-6 anos.
Os resultados desta análise demonstram que, com a maior escolaridade dos pais,
aumenta a sua divergência face às avaliações dos profissionais e que o nível
socioeconómico dos pais influencia as oportunidades de desenvolvimento da criança
com PEA, face aos resultados de avaliação mais positivos das crianças cujo nível
socioeconómico dos pais é mais elevado. Autism Spectrum Disorders (ASD) are characterized by a triad of difficulties in relation to interaction, communication and reduced interest. However, in recent years, research has also given particular relevance to changes in the Sensory Processing of these children which indicate that between 42 and 96% of children with ASD exhibit this type of dysfunction. Therefore, it is in this context that this research aims to construct an instrument to evaluate the traditional triad featuring children with ASD (interaction, communication and behaviour and interests), and enriching this review with the inclusion of a new domain: Sensory Processing. Given the lack of sufficient assessment tools validated for this population, the overall aim of the thesis focuses on the construction, adaptation and validation of a tool for assessment and intervention of children with ASD, aged 3-6 years. Moreover, it presents the procedures and results of the successive phases of construction of the instrument, the initial studies among parents, professionals and experts in the sense of their understanding of the items and, their relevance in the assessment of the various dimensions of the scale. After this phase, a pilot study of eleven families to identify the levels of agreement in the classification of checked items (indicators) in several dimensions by professionals with several years of professional experience with children with ASD was carried out, which describes the results of an analysis of the quantitative nature of the items, resulting from the application of the first version of the scale. Seeking to identify the greatest number of children with ASD at the national level, the collaboration by various institutions distributed across the five geographical areas of the country (North, Centre, Lisbon, Alentejo and Algarve) was sought. A national sample of 260 children with ASD was obtained; 260 scales completed by professionals that accompany these children, and 136 instruments simultaneously filled out by families. In terms of conclusions, the analysis performed on four factors or dimensions for analysis, showed that some items were distributed with approximate overruns on two factors simultaneously (usually in factors I and II) giving rise to the "merger" between "Social Interaction "and" verbal and nonverbal Communication ", initially considered two independent domains in this research and, which presents itself as an interesting result in light of current research on ASD results. This result allowed for the annexation of items belonging to the two initial domains and proposes a "new" dimension which we called: Social Communication, as envisaged in the new diagnostic criteria listed in the DSM 5. Still relevant to the construction of the scale, the results reinforce the relevance and autonomous existence of the Sensory Processing domain, included in the scale, compared to traditional domains (social interaction, communication, behaviour and reduced interest. The analysis of agreement between the developmental profile of the child with ASD aged 3-6 years reported by parents and professionals, reveals that there are differences in the assessment of children by parents and professionals with the best scores given by families especially in the area of Social Communication. It is also worth noting the influence of socioeconomic variables and education levels of parents in the definition of the profile evaluation of the child with ASD, aged 3-6 years. The results of this analysis demonstrate that with higher parental education levels, there is an increase in divergence from the assessment of professionals and also, that the socioeconomic level of parents greatly influence the development opportunities of children with ASD. |
