Resumo(s): | O objetivo deste projeto baseia-se na avaliação do efeito da instilação de fenilefrina em
diversos parâmetros oculares para averiguar a existência ou não de alterações nos mesmos. Essa
avaliação foi realizada com o propósito de comparação entre dois grupos de diferentes faixas etárias
(um grupo de voluntários jovens e um grupo de voluntários présbitas), ao longo de um período de
180 minutos, após a instilação da primeira gota do colírio. Também foi realizada uma avaliação de
todos os parâmetros no olho contralateral, para averiguar eventuais efeitos de enervação pupilar
simpática adversos à instilação do fármaco.
Foram avaliados 20 voluntários, 10 no grupo de jovens e 10 no grupo de présbitas, que
realizaram medidas de baseline e aos 15, 30, 90 e 180 minutos posteriores ao da instilação da
primeira gota de fenilefrina. Os parâmetros avaliados foram os seguintes: refração objetiva,
aberrometria, tonometria, paquimetria e biomecânica corneal, diâmetro pupilar (para diferentes
condições de iluminação), a tomografia da córnea e do segmento anterior e inquéritos subjetivos de
forma a avaliar sintomatologia (descrição e duração) inerente à instilação de fenilefrina tópica.
Os resultados experimentais indicaram o pico de ação da fenilefrina aos 90 minutos, no
entanto, estes já se mostraram significativos a partir dos 20 a 30 minutos iniciais. Ambos os grupos
de estudo, jovens e présbitas, atingiram um máximo de dilatação pupilar de cerca de 8 mm, sendo
que, no grupo présbitas, verificou-se um diferencial de diâmetro pupilar entre a baseline e os 90
minutos, superior em 2 mm, comparativamente ao grupo jovens.
Apesar de se obter uma dilatação aos 30 minutos, aos 90 minutos ainda se consegue
ganhar mais 1 mm de diâmetro pupilar, o que poderá ser importante nalgumas aplicações clínicas,
tal como, na oftalmoscopia. Aos 15 minutos de estudo verificou-se, no Grupo Jovens, um menor
valor de diâmetro pupilar face à baseline, possivelmente devido a uma adaptação à menor
iluminação da sala, apenas conseguida nos 15 minutos posteriores. No Grupo Présbitas, não se
verificou a mesma tendência, o que sugere que, no Grupo Jovens, haja uma maior latência na
absorção do fármaco, devido a barreiras biológicas mais resistentes. Para além do efeito midriático
pretendido, diversos parâmetros óticos e estruturais do olho sofreram alterações após a instilação
de fenilefrina, embora geralmente muito ligeiros. Futuros estudos com amostras mais alargadas são
necessários para confirmar as hipóteses lançadas neste manuscrito. The goal of this project relies on evaluating the effect of phenylephrine instillation in various ocular parameters to ascertain whether or not changes occur in them. The evaluation was performed with the purpose of comparison between two groups of different age groups (a group of young volunteers and a group of presbyopic volunteers) over a period of 180 minutes after the instillation of the first collyrium drop. It was also performed an evaluation of all the parameters for the contralateral eye to verify possible side effects in pupillary sympathetic enervation due to the drug instillation. Twenty volunteers were evaluated, 10 in the youth group and 10 patients in the presbyopic group, patients underwent baseline measurements and at 15, 30, 90 and 180 minutes after the instillation of the first drop of phenylephrine. The parameters evaluated were as follow: objective refraction, wavefront analysis, tonometry, pachymetry and corneal biomechanics, pupillary diameter (for different lighting conditions), computed tomography of the cornea and anterior segment and subjective surveys to assess the symptomatology (description and duration) inherent to the topical instillation of phenylephrine. The experimental results indicated the peak effect of phenylephrine at 90 minutes, however, these have proven significant from the initial 20 to 30 minutes. Both study groups, young and presbyopic, peaked pupil dilation of about 8 mm, while in presbyopic group there was a difference in pupil diameter between baseline and 90 minutes greater than 2 mm compared to the young group. Although obtaining a dilation at 30 minutes, after 90 minutes it still increases 1mm more in pupillary diameter, which may be important in some clinical applications, such as on ophthalmoscopy. At 15 minutes of observation it was found in the youth group, a lower value compared with baseline pupil diameter, possibly due to an adaptation to the low room lighting, achieved only 15 minutes later. In the presbyopic group, the same tendency was not verified, which suggests that, in the youth group, there is a higher latency in the drug absorption, due to stronger biological barriers. In addition to the desired mydriatic effect, various structural and optical parameters of the eye suffered minor changes after the phenylephrine instillation. Future studies with broader samples are needed to confirm the hypothesis launched in this manuscript. |
